    Alzheimer's Association of South Central WI helps caregivers take care of themselves as well as provide a quality of life for the person with Alzheimer's disease or related dementia. It directs families to local community resources and services, provides a telephone help-line, counseling, care management, support groups, a lending library, training and education.

    The Wisconsin APDA Information and Referral Center responds to the particular needs of those affected by Parkinson's disease, and of their caregivers, through education, referral, support and public awareness programs. APDA helps educate the community about Parkinson’s disease since many people go undiagnosed.

    Area Agency on Aging of Dane County develops policies, prioritizes funding, and oversees county aging services. It collaborates with local organizations, elected representatives, public / private planners, and service providers, coordinates the Senior Nutrition Program and is the lead agency for elder abuse investigation in Dane County.

    Eldercare Locator is especially useful for long-distance caregivers because it provides state and local information on home care, assisted living programs, and nursing homes.

    Parish Nurses work in congregations and communities to assist people with body, mind, spirit and well-being. Their work is to assist individuals to reach their optimal health and then in staying well by using all resources available to them. The main components of parish nursing are: health educator, personal health counselor, referral agent & liaison with congregational and community resources, developer of support groups, trainer of volunteers, integrator of faith & health, and health advocate.

    Personal Care Manager, Inc. coordinates care for the elderly and disabled persons.
  • Respite Care Team Ministry - (608) 833-4800
    Respite Care Team Ministry is a team model of volunteers coordinated by Catholic Charities Aging Services, in partnership with Oakwood Village and area congregations. The teams provide various in-home, non-medical assistance, including respite. Services are provided free of charge, regardless of race, religious belief, or economic status.

    Senior Hotline to Help (Dane County Department of Human Services) is an information & referral line for anyone over 60 in Dane County.
Power of Attorney
  • Two copies of the Power of Attorney for Health Care form are available free to anyone who sends a stamped self-addressed business-size envelope to: Power of Attorney, Division of Public Health, PO Box 309, Madison, WI 53703. This is not yet available on the internet.
