All filings and requests through the mail require a self addressed stamped envelope to return incorrect and/or processed paperwork and receipts to you. Please ensure all required documents are properly notarized before filing with the court.

Criminal / Traffic Forms

Most Criminal/Traffic Court Forms can be found on the Circuit Court forms website on the Wisconsin Court System's website. These are mandatory forms and if there is a form on this page to meet your needs, you MUST use that form. However, there are a few Dane County-specific forms for Criminal or Traffic Court. If you do not see the form you need listed below, please contact the Clerk of Courts at the location above or the Dane County Legal Resource Center at (608) 266-6316 for assistance.
