How to Obtain An Identification Card To Vote
If you are eligible to vote in Wisconsin and do not currently hold a Wisconsin driver’s license
or Wisconsin identification card, you may obtain a free identification card from the Wisconsin
Department of Transportation Division of Motor Vehicles (DMV). You will need to check a box on
the application form indicating that the identification card is needed for voting:

The DMV will require you to present the following documents in order to obtain the
Wisconsin identification card required for voting:
- Proof of name and date of birth (a certified United States birth certificate, valid passport, or certificate of naturalization)
- Proof of identity (Social Security card, military papers, marriage papers)
- Proof of Wisconsin residency (utility bill, paystub, mortgage)
- Proof of United States citizenship
- Social Security number
The Madison East DMV service center, 2001 Bartillon Drive, is open:
8:30 – 4:30 Mondays, Tuesdays, and Fridays
8 – 4 Wednesdays,
and 10 – 5:30 Thursdays.
The Madison West DMV service center, 4802 Sheboygan Avenue, is open:
8:30 – 4:30 Mondays, Tuesdays, and Fridays
10 – 5:30 Wednesdays,
and 8 – 4 Thursdays.
The Madison Odana Renewal Center, 6514 Odana Road, Suite 4, is open for license and
identification card renewals only:
10 – 6 Tuesdays, Thursdays, and Fridays
10:15 – 5:30 Wednesdays,
and 8 – 4 Saturdays.

CERTIFIED BIRTH CERTIFICATES: In order to obtain a free identification card for voting
purposes, you will need to present a certified birth certificate or passport to the DMV.
There is a fee for obtaining a certified copy of a birth certificate.
IF YOU WERE BORN DANE COUNTY: Contact the Dane County Register of Deeds, 210 Martin Luther

There is a $20 fee and you will need to provide one of the following forms of identification:
- Wisconsin driver’s license or Wisconsin ID card with photo or current college or high school identification card with photo
- Two documents with your name and address (utility bill, WI hunting or fishing license, government-issued photo ID badge, U.S. passport, check/bank book, health insurance card, recently signed and dated lease, traffic ticket)
- Family member/friend with photo ID who will corroborate for you
If you were born in another WISCONSIN county:
Contact the Wisconsin Department of Health Services, 1 W. Wilson Street, Room 158,
dhs.wisconsin.gov/vitalrecords.
There is a $20 fee and you will need to bring one of the following forms of identification:
- Wisconsin driver’s license/photo ID or out-of-state license/photo ID
- Two of the following: Government-issued employee identification badge with photo, US passport, checkbook or bankbook, Major credit card, Health insurance card, recently dated and signed lease, utility bill, traffic ticket
